Hospital Readmission Reduction Program (HRRP) Revisited: Outcomes, Risks, and Benefits

Thursday, February 26, 2026

6:00 pm - 7:00 pm ET

Hospital readmissions remain a major focus in healthcare—and case managers play a critical role in reducing them. Join us for a one-hour educational webinar Hospital Readmission Reduction Program (HRRP) Revisited: Outcomes, Risks, and Benefits, created specifically for professional case managers.

This practical session takes a fresh look at HRRP—how it’s evolved, what the data really shows, and what’s working (and not working) in real-world practice. We’ll explore evidence-based strategies for reducing avoidable readmissions, the role of care coordination and care transitions, and how patient engagement and social factors influence outcomes well beyond discharge.

Whether you work in acute care, post-acute settings, or population health, this webinar offers practical insights you can connect directly to your day-to-day practice—and reinforces why strong case management matters more than ever.
